A fillable .pdf file of the Texas Official WDI Report (SPCS/T-5) (09/01/2020 version) customized with your company information will allow you to easily fill out the report in the field using an iPad, laptop or other hand held multi-touch tablet. Completed WDI reports can be saved, printed and/or emailed to your customer. There are no monthly or annual fees. The programming fee is a one-time charge.
This three 8-1/2" x 11" page version has been reviewed and approved by Texas Department of Agriculture Structural Pest Control Service. The first page includes yellow highlighted areas as required by TDA. Form SPCS/T-5 Texas Official Wood Destroying Insect Report was revised 9/1/2020.
